Referring a Patient

Mind-Care is a multidisciplinary clinic based in Charlestown, NSW, providing psychologypsychiatry and neurology services. 

We welcome referrals from GPs, psychiatrists, medical specialists and support coordinators for patients requiring psychological therapy, psychiatric assessment, neurological assessment or multidisciplinary care.

Appointments are available in person at our Charlestown clinic and via secure telehealth across Australia.

    Referrals are welcome from:

    • Support coordinators and NDIS plan managers 
    • Hospital and community health services 

    Mind-Care is a registered and accredited provider of NDIS services and can support participants through structured Service Agreements and Support Plans where appropriate.

    If you are unsure whether a referral is appropriate, our team is happy to discuss the case prior to referral. 

    Patients who may benefit from referral to Mind-Care include those requiring: 

    • Psychological therapy 
    • Multidisciplinary mental health care 
    • Psychiatric assessment or medication review 
    • Neurological assessment 
    • Support while awaiting specialist care 
    • Telehealth access due to geographic or mobility barriers 

    Mind-Care’s multidisciplinary model allows patients to access coordinated care when psychological, psychiatric, or neurological factors are involved. 

    1) Referral received
    Our administration team confirms receipt of the referral. 

    2) Clinical review
    The referral is reviewed to determine the most appropriate clinician or service. 

    3) Patient contact
    Our team contacts the patient to arrange a suitable appointment. 

    4) Communication with referrer
    Relevant updates and reports are provided to the referrer where appropriate.
